Abstract

Patients with a far advanced incurable disease for whom appropriate care in their home is no longer ensured but admission to a hospital setting, e. g. a palliative care unit, is not required, are entitled to hospice care according to § 39 a German Social Code Book V in a hospice. Inpatient hospice care aims at improving and maintaining, respectively, the best possible quality of life by means of competent symptom control including palliative medical and nursing care. In 2005, previous to a legal regulation of remuneration for specialised palliative outpatient care teams, a survey was undertaken among the heads of hospices (n = 33) and physicians (n = 58) working in inpatient hospices in North Rhine Westphalia, the state with the highest density of hospices in Germany. The survey aimed to explore matters of organisation and quality of medical care in this setting. A high percentage of the respondents rated the quality of medical care as good in this region. Most respondents considered the involvement of a physician with qualification in palliative care as the best possible means for the provision of appropriate care. Identified deficits of care included psycho-social and communicative skills of the involved physicians, and issues related to pain control. Work-related difficulties reported by the physicians were lack of time for the care of hospice inpatients and a not cost-covering remuneration. The results of this survey emphasize the positive impact that the new legal regulations on specialised palliative care in the outpatient setting will have on achieving an essential goal in the care for the critically ill and the dying. Cooperation of the physicians working in hospices with specialised palliative care teams will broaden the basis for delivery of a needs-oriented, high-quality medical care in inpatient hospices in all regions. Above that the presented data can be used as baseline results for a comparative study after implementation of specialised outpatient palliative care teams with remuneration contracts according to the new law in Germany.
