Abstract

Descemet Stripping OnlyDescemet Stripping Only (DSO) (DSO), alternatively termed Descemetorhexis Without Endothelial Keratoplasty (DWEKDescemetorhexis Without Endothelial Keratoplasty (DWEK)), is a relatively novel surgical approach for the treatmentTreatmentof FuchsEndothelial corneal diseases endothelial corneal dystrophyCorneal dystrophy. The underlying rationale of this procedure is to remove the central area of diseased endothelial cell-Descemet membrane complex to allow the healthier peripheral cells to repopulate the bare corneal stroma, avoiding the need to insert a donor graft. Thus, DSODescemet Stripping Only (DSO) is currently indicated in patients with early Fuchs dystrophyFuchs dystrophy, central guttaeGuttae and a healthy endothelial cell reservoir in the periphery. Current evidence suggests that the size of the central descemetorhexisDescemetorhexis should be limited to 4 mm, allowing central migration of peripheral endothelial cells, resolution of corneal edemaCorneal edema and increase in visual acuity in the majority of treated patients. Time to corneal clearance and visual recovery may be accelerated by the use of topical Rho kinase (ROCK) inhibitors as adjunctive treatmentTreatmentafter DSODescemet Stripping Only (DSO). However, long-term clinical studies have to show whether DSODescemet Stripping Only (DSO) can provide lasting visual outcomes similar to Descemet Membrane Endothelial KeratoplastyKeratoplasty (DMEKDMEK), or whether recurrence of guttaeGuttae and endothelial dysfunction will occur.
