Abstract

Introduction: Red blood cell concentrates (RBCs) as the only therapeutic option for severe anemia are validated for storage times of up to 49 days. In order to eliminate storage-related degradation products, the RBCs can be reprocessed in accordance with the hemotherapy guidelines for the collection of blood and blood components. The aim of the in vitro study is to evaluate the processing by means of mechanical autotransfusion (MAT) in the setting of pediatric cardiac medicine. Material and methods: In a prospective, randomized observational study, 25 RBCs with a storage time of 37 days were washed in equal parts with a hemofiltration solution using four different MAT devices and a blood bag centrifuge. Samples were taken at the measurement times M0 before washing, M1 after washing and M2 24 h after washing. The criteria for the preparation of RBCs according to the hemotherapy guidelines of 2023 (hematocrit: 50–75%, total Hb: > 40g/RBC, hemolysis rate: < 0.8%, total protein: < 0.5 g/unit, microbiological: no microbial growth) were analyzed. In the statistical analysis, the question of the equivalence of MAT to transfusion preparation by a blood bag centrifuge was investigated. Results: The test parameters of the current guidelines for the washing of RBCs were achieved for both the blood bag centrifuge and the four MAT devices. Conclusion: Packed red blood cells can be regenerated using conventional MAT systems in accordance with the guidelines. A logistically complex preparation via transfusion medicine is possible in favor of a practicable, solution-oriented application in everyday clinical practice, without compromising quality. The advantage of intraoperative MAT processing lies in the rapid availability of devices and washed RBCs and the additional processing of suction or wound blood.
