A 26-year-old patient with eccrine porocarcinoma developed regional lymph node metastasis. By this unusual constellation of age and course of metastasized adnexal carcinoma, we discuss the management in primary care, possible imaging, follow-up and therapy in the metastasized setting. Rare adnexal carcinomas such as eccrine porocarcinoma should preferably be treated by an interdisciplinary team, in an individualized manner and in close consultation with the patient.
