Abstract

Introduction: Chronic postoperative inguinal pain is a complication of inguinal hernia repair that occurs in 10 % to 12 % of cases; most of them require a reoperation. During the COVID-19 pandemic, many countries restricted elective surgical procedures to prioritize emergencies. We present a case of chronic postoperative inguinal pain where non-operative strategies were used for its treatment, while surgical activities were reactivated due to hospital conversion. Case report: A 57-year-old male with obesity and comorbidities, with intense postoperative right inguinal pain of 14 months of evolution. No recurrence were observed on physical and ultrasonographic examination. Dermatome mapping (DMT) was performed, appreciating hyperalgesia along the path of the right ilioinguinal nerve. Since at that time the hospital operating room was disabled during the COVID-19 pandemic, the patient was administered an intravenous solution of ketamine (0.1 mg/kg) plus dexamethasone (8 mg), diazepam (10 mg) and metamizole (1 g) during four outpatient sessions, with an interval of 15 days between each session; and oral pregabalin (75 mg daily) for 50 days, presenting complete remission of pain at the end of treatment. Two months later, the patient reported a recurrence of pain, so when the operating room was rehabilitated, surgical exploration and meshoma removal were performed, with pain relief. Discussion: The use of ketamine may be useful to treat CPIP, when elective surgeries must be postponed, such as during the COVID-19 pandemic, but relapse seems to be the norm and surgery the definitive option. Mesh removal seems to have a positive effect on CPIP improvement after inguinal hernioplasty.
