Abstract

This article outlines the development and significance of certified gynecological cancer centers and dysplasia units in Germany. Historically, oncological care lacked standardization, leading to inferior survival rates until the early 2000s. In response, the German Cancer Society (DKG), together with professional societies, established a comprehensive certification program in 2003, now comprising more than 2400 centers. Since 2008, specialized gynecological cancer centers have ensured a continuous care pathway from prevention and diagnosis through therapy to follow-up. Central to this approach is the “Oncology Cycle” (Plan–Do–Check–Act), systematically linking clinical guidelines, quality indicators (QIs), and cancer registries. Using cervical and vulvar cancer as examples, the article illustrates how QIs are developed, implemented, and updated. Certified dysplasia consultations and units form the entry point of the care chain, providing standardized diagnostic access and now being widely available across the country. Another key focus is the integration of personalized medicine through genetic testing (e.g., BRCA, Lynch syndrome) and molecular tumor boards within centers for personalized oncology. The incorporation of molecular tumor boards further enhances individualized treatment decisions and the implementation of tailored recommendations. Overall, certification ensures verifiable quality and measurable clinical effectiveness. Priorities for future development include consistent QI-driven governance, nationwide integration of dysplasia units, systematic adoption of personalized medicine, and the inclusion of structured quality standards in hospital planning and reimbursement systems.
