Although approximately 500 hospitals in Germany currently provide evidence-based care for the invasive elimination of large abdominal aortic aneurysms (5.0 cm in women and 5.5 cm in men), and this topic is of central importance in international vascular surgery, there is consensus that complementary Best Medical Treatment before and after any invasive procedure significantly influences long-term treatment outcomes. Numerous risk factors, many of which are modifiable, influence the development of pathological dilations of the aorta and are directly associated with the progression of the disease as well as accompanying cardiovascular comorbidities. In addition to smoking cessation, optimal drug therapy and risk factor optimization are therefore considered important pillars of abdominal aortic aneurysm therapy, although an evidence-based specific drug therapy to prevent aneurysm growth is not yet available. Adherence to numerous treatment recommendations is of great importance for long-term rupture-free survival and cardiovascular health. However, recommendations for physical rest are not based on robust evidence and may, on the contrary, lead to poorer cardiovascular outcomes. This layman-friendly educational article summarizes the evidence-based recommendations for the development and prevention of abdominal aortic aneurysms.
