Barrett's esophagus is still a confusing diagnosis which depends on many factors. The importance of the underlying mechanisms and the histological findings are not clearly defined yet. Besides the well-known interobserver gap in the diagnosis of Barrett's esophagus, the increasing number of differences between diagnosis by biopsy and endoscopic or surgical specimen is a rather new phenomenon. Nevertheless, histological diagnosis before and after endoscopic or surgical intervention is still of great importance for planning further therapeutic procedures. © 2012 S. Karger GmbH, Freiburg.
