Abstract

Background: Behavioral changes often involve a conflict between aversive short-term and positive long-term consequences for the patients. These conflicts often have a negative impact on the accomplishment of behavioral changes planned and agreed upon in therapy. The objective of this study is to clarify whether the probability of patients accomplishing planned behavior changes can be increased by fostering motivational and volitional pro-cesses (decision making). Methods: Based on general motivational approaches, the prescriptive commitment model 'PRAEMO' was developed. From this model the commitment enhancing intervention 'EFI' was deduced and its efficacy exemplarily evaluated in a sample of 63 inpatients with assertiveness skills deficits in a randomized controlled design. Results: The rate of patients, who after discharge actually performed an assertiveness behavior that had been planned in therapy, was significantly higher in the decision-enhancing condition EFI (91%) than in the control condition with a conventional cognitive-behavioral intervention (66%). Conclusions: Starting from a thorough analysis of the commitment process, effective commitment-enhancing interventions can be deduced. These help patients to effectively accomplish planned behavior changes. © 2005 S. Karger GmbH.
