Abstract

Invasive diagnostic coronary procedures – which in addition to invasive angiography also include intracoronary imaging and measurement of the fractional flow reserve (FFR) – as well as interventional and surgical coronary revascularization are effective measures for the diagnostic workup and treatment of patients with coronary artery disease. Nevertheless, they are invasive procedures with corresponding cost and effort, and a nonnegligible complication rate. Hence, the indication to perform invasive coronary diagnostic procedures and revascularization must be made under careful consideration of the patient’s clinical situation, the results of noninvasive testing, and comorbidities, particularly in patients with known or suspected stable coronary artery disease, rather than unstable patients or those with acute coronary syndromes. In this position statement, clinical indications for invasive diagnostic coronary procedures and for coronary revascularization are summarized, taking into account, wherever possible, respective Clinical Practice Guidelines published by the European Society of Cardiology and endorsed by the German Cardiac Society.
