Abstract

Objective: The family practice is said to be the ideal setting to initiate smoking cessation. The German nationwide study "ASP-kardio" delivers current data concerning the status quo of smoking cessation support offered in German family practices according to the "5 A's strategy". Methods: 4000 general practitioners were questioned in a representative survey between October 2011 and March 2012. Before that, the questionnaire had been revised by experts, validated by cognitive interviews and tested in a pilot study. Results: 89% of the general practitioners assessed tobacco consumption among their patients ("Assess"), 82% advised smokers to stop smoking ("Advise") and 12% fixed a written agreement on smoking cessation objectives as a matter of routine. Assistance ("Assist") in the form of a brief intervention was much more commonly offered (72%) than in the form of providing information leaflets (33%) or offering a smoking cessation therapy (27%). About half of the questioned general practitioners (54%) arranged a follow-up appointment to check if patients succeeded in quitting smoking ("Arrange"). The support offered depended on the general practitioner himself/herself, on the condition and location of the practice as well as on patient specific factors. In the federal states (Bundesländer) located in the northeast of Germany, the support offered was below average. Eight out of ten general practitioners (77%) said that their methods were not successful. Conclusion: The suboptimal and unequally distributed care of smokers offering appropriate smoking cessation measures in Germany's primary care becomes evident. © 2014 Verlag Hans Huber, Hogrefe AG.
