Abstract

The aim of the pharmacotherapy of functional obstipation is initially to relieve acute constipation (disimpaction) and then to ensure regular pain-free bowel emptying by means of stool-softening long-term treatment. The scientific state of the art of acute pharmacotherapy of constipation with enemas as well as with oral macrogol is analyzed with respect to efficacy and safety. For acute treatment enemas containing phosphate are often used, which in the past have occasionally led to severe complications, including death. These severe complications are exemplified in a case study of a 16-year-old girl. The regulatory background and current changes with new contraindications are presented as they are important for prescription and reimbursement. An alternative safe and appropriate pharmacotherapy for acute constipation is the oral administration of high-dose macrogol and the indications, dose, and data on efficacy and safety are presented. Oral administration of macrogol is a good alternative to enemas, when treating children for acute obstipation. Enemas containing phosphate are dangerous, particularly in young children under 6 years of age if they are not eliminated in a timely manner.
