Abstract

Fractures of the acetabulum occur in a bimodal distribution. In young patients, fractures of the acetabulum are predominantly induced by high-energy trauma, whereas in the elderly population, even minor trauma can lead to fractures of the acetabulum due to osteoporotic bone. The exact classification of the fracture is important for the assessment of the stability, and thus for the decision for conservative or operative therapy. While fractures of the posterior column and posterior wall are treated by the dorsal approach, there is a trend towards the use of the intrapelvic and pararectus approach for most other fracture types. Basically, the restoration of the joint surface is of major importance to avoid the incidence of posttraumatic osteoarthritis, while tissue trophic and vascularization must not be affected. In certain cases, minimally invasive approaches may be reasonable to prepare secondary arthroplasty. Primary arthroplasty may be performed in those cases with defects that cannot be restored anymore, severe osteoporosis or addititve fractures of the proximal femur. It may be necessary to apply additive osteosynthetic stabilisation. For younger patients, anatomic reconstruction is of major importance, whereas in geriatric patients, the demand for early mobilisation plays a crucial role regarding the decision of the therapeutic strategy.
