Konzeption und Umsetzung eines Qualitätsmanagements für den ambulanten Rehabilitationssport bei Osteoporose in Bayern

Abstract

The primary setting of osteoporosis therapy and fracture prevention by means of exercise in Germany is 'Rehabilitationssport' and 'Funktionstraining'. Based on German law, people with osteoporosis are entitled to prescribed medical rehabilitation sports. However, the weak point of the German structure is the lack of mandatory quality management (QM) with respect to process (i.e. proper application of a validated exercise protocol) and result (i.e. validation and documentation of exercise-induced changes). Thus, the aim of the present project, that was conducted in close cooperation with the Bavarian Disabled and Rehabilitation Sport organisations (BVS Bayern e.V.), was (a) to provide a reliable concept to transfer a validated exercise programme in clinical praxis of Rehabilitationssport-groups, (b) to develop and implement a QM-concept that specifically address process and result-quality aspects and (c) to provide concepts for the ongoing adaptation and enhancement of the exercise protocol. After development of the QM-concept and a 12 months test trial with 10 Rehabilitationssport- groups, all members of BVS Bayern, we observed a high transferability of the validated exercise protocol in the clinical practice of Rehabilitationssport-groups. After ongoing adjustments of the QM-concept during this test phase, a quality signet for 'exercise in the rehabilitation of osteoporosis' was released by the BVS Bayern and the Bavarian Sports Physicians Association that is awarded for participation in this quality concept. However, due to the acute situation in Germany, that the demand for rehabilitation sports for osteoporosis exceeds the current supply, most rehabilitation sports groups might feel that there is no need for enhanced, demanding and at present not mandatory quality management concepts. Thus, in summary the current situation in Bavaria/Germany is, that, although a sophisticated concept for the secondary and tertiary prevention of osteoporosis by means of exercise is already being provided and validated, the general interest and motivation of the service suppliers (i.e. rehabilitation sport groups) to apply this concept is low.
