Abstract

Innovative care concepts for multimorbid patients should be oriented towards eliminating healthcare deficits. These are inappropriate care, different health care providers acting in isolation, failure to involve family caregivers in intervention planning, regional differences in healthcare and failure to take into account the specific care needs of people with dementia. An outline for an innovative healthcare concept is presented.
