Abstract

Background: Postoperative pain after anorectal surgery is a frequent challenge. The block of the pudendal nerve is an established method for intraoperative and postoperative pain management. Objective: The aim of this investigation was to assess pain relief and the duration of the pudendal nerve block as well as the influencing factors. In addition, the question to be answered was whether a repetition of the injection has an influence on its effectiveness. Material and method: A total of 301 patients with anorectal surgery and painful diseases of the anorectum were included in the study and received an injection of 10 ml bupivacaine 0.75% + 75 µg clonidine per side preoperatively and postoperatively at the time points day 0, (1), 3 and if necessary 2 times/week. In addition to biometric data, pain intensity before and after application, pain reduction, pain-free interval, and number of applications were recorded for each patient. Results: The average pain reduction (as a reduction in the visual analogue scale) was 2.1 in men, 2.5 in women, and 2.3 on average (p < 0.0001). The average pain relief lasted 51.5 h and older age (p = 0.009) and male gender (p = 0.025) were associated with longer duration of the block. The frequency of the administration of the block had no effect on the duration of the pain relief as well as the intensity of the pudendal nerve block. Conclusion: The pudendal nerve block is an effective and easy to perform method for postoperative pain therapy after anorectal surgery, which can be used repeatedly and effectively.
