Abstract
The term cancer of unknown primary (CUP) syndrome is used to describe malignancies in which a complete diagnostic workup detects metastases in the absence of an identifiable primary tumor. Most patients diagnosed with CUP have an unfavorable prognosis. Nevertheless, it is important to identify subsets of patients for whom specific treatment offers the chance of long-term survival or even full recovery. Axillary metastasis of adenocarcinoma in this circumstance predominantly originates from an undetected breast tumor. Only further development of diagnostic tools and treatment protocols will enable improvement of the poor prognosis. Specific molecular treatment strategies have shown promising results. Techniques such as magnetic resonance imaging and positron emission tomography can help detect previously overlooked tumors of the breast as well as help in initiating optimized therapy for these patients. Cystosarcoma phyllodes (CP) is an extremely rare form of breast cancer with an unpredictable clinical course. The histological characteristics of this neoplasm have not been proved to offer much in estimating prognosis of these patients. Early diagnosis and margin-free surgery are prognostically relevant. Independent of its histopathological behavior, CP is a tumor that is difficult to treat. Meticulous follow-up is mandatory to manage possible recurrence of the neoplasm. © 2009 Springer Medizin Verlag.
