Abstract

A major challenge in the emergency department (ED) is to identify critical ill patients immediately at presentation. "Triage" is a structured approach usually performed by trained emergency nurses to identify ED patients at risk, categorize them and prioritize diagnostics and treatment. 5-level triage systems such as the Emergency Severity Index (ESI), Manchester Triage System (MTS), Canadian Triage and Acuity Scale and Australian Triage System provide acceptable validity and reliability supporting adapted guideline-based therapy within the ED. During last decades ESI and MTS have been established in German EDs and provide opportunities for further professionalization of emergency care. In addition to the categorization of medical urgency and emergency, triage systems offer important chances for the improvement of patient streaming in the ED: 1. Patients can be transferred to adequate treatment area within the ED. 2. Management of ED crowding can be improved and provides assistance for management of ED care. 3. The interface among preclinical care and hospital is structured and provide transparency and traceability for ED management. 4. In addition, quality management can be instituted. However, some limitation of current triage models has to be kept in mind. Special patient populations, e.g., elderly patients, are at considerable risk for under- or overtriage. Thus, geriatric experience is mandatory for triage nurses to prevent inaccurate assessment of the urgency of treatment. In summary, structured nurse-driven triage has become an integral part in modern ED and steps for further development may include the use of such systems in preclinical care. © 2014 Dustri-Verlag Dr. Karl Feistle.
