Abstract

Background: The electronic cigarette (e-cigarette) is an innovation which is rapidly gaining popularity around the world. Initial systematic reviews of the potential risks of e-cigarettes (nausea; dizziness; respiratory tract irritations; allergic reactions; brain and heart-circulation system damage; nicotine addiction and increased difficulty in quitting among cigarette smokers) have already been published. Aim: This paper is one of the first to investigate e-cigarette use and risk perception among adolescents. Study design: Between October 2014 and March 2015, 840 seventh- and eighth-grade school children from the Rhine-Neckar metropolitan region were anonymously interviewed. Results: Almost all of the school children questioned knew about e-cigarettes (98.0 %) and 16.2 % of those asked had used an e-cigarette at least once. The majority of the adolescents who had used e-cigarettes (namely 83 % of the e-cigarette users) did not smoke tobacco cigarettes. The majority of adolescents questioned perceived e-cigarettes which contain nicotine to be addictive (acceptance rating 88 %). Cluster analyses identified three different groups of adolescents with regard to risk perception: Those “generally concerned” (25 % of all adolescents questioned), those “concerned about long-term effects” (59 %), and those “generally unconcerned”(17 % of all adolescents questioned). The last cluster included a higher percentage of males, students from “Real-” and “Haupt-” (professional secondary) schools, and adolescents with migrant backgrounds. Implications: Analyses such as the one presented here make early monitoring possible and allow the development of data-based prevention measures.
